Services

We are a 139-bed Medicare and Medi-cal certified and licensed Skilling Nursing Facility in the heart of Sacramento, California. Established January 16, 1997, our mission is to provide the highest quality post-acute care while meeting the evolving needs of our healthcare community.

Rehabilitation Therapy

We provide comprehensive, in-house physical, occupational, speech-language, and restorative therapy to help residents achieve their highest level of function. Short- and long-term residents receive personalized care and treatments as needed to support mobility, strength, communication, daily functioning, and overall well-being.

Our services include post-acute care, direct admissions from home, respite care, and hospice care, providing compassionate support tailored to each individual's needs.

It was hard adjusting to a new environment. My independence was gone and I lost a lot of weight. I had to drop out of school. Staying at Asbury Park changed my life. I became independent and met cool people. Karen from activities gave me a reason to keep smiling. The CNAs were very attentive. Social services helped me get my mind back on the right track and got my social security back. Thank you Asbury Park -David Edwards 🥰

I loved my stay. I'm leaving today and I'm sad because I want to stay with these beautiful women. Teanna in social services and nurse Mary are the most kind staff I came across. The food at this place was soon much better tha the food at the previous facility I was in. I will miss this place. Thank you for getting me stronger Asbury.

My Mother was admitted to Asbury Park Nursing & Rehabilitation to receive PT/OHT before she could go home. Everyone, from Management to Housekeeping care about the patients under their care. Mom always complemented how good almost all the food was. There was only 1 dish that was too spicy for her. Thank you Woodside for caring for my Mom. Christine

My mother received wonderful care during her stay. The facility was clean. The staff was kind and helpful and she loved participating in bingo and relaxing on the beautiful front patio! Most of the people are good and made everything exciting to be here! It was fun meeting the activity staff and doing karaoke!!! Eugene and Karen sang Karaoke so good!!! Kaylin is a nice person. they're always friendly and greeting people! so much fun here!!!

Lucy has been diligent in getting my uncle's medicare reinstated thankfully. Also, Kiana has. been diligent in assuring my uncle's medical appointment regarding his kidneys was followed up.

My neighbor and dear friend was recently admitted to this facility from Kaiser, and I cannot say enough about the exceptional support we received. The admissions team was incredibly helpful throughout her entire stay, making what could have been a stressful experience feel smooth and reassuring. We especially want to recognize Pauline and Debi for their kindness, compassion, and dedication. They were always available—throughout the day and even on weekends—to answer every question with patience and care. Debi was available when I needed her on the weekend. I highly recommend this facility to anyone who is new to skilled nursing and in need of guidance, support, and truly caring professionals.

I’ve had the opportunity to witness the level of care and service provided at Asbury Park Nursing & Rehabilitation, and it truly stands out. The staff consistently demonstrate professionalism, compassion, and a genuine commitment to the residents they serve. After our father suffered a stroke, Kaiser provided us with a list of facilities that could accept him, after touring several of them, which some truly disappointing, we are grateful we chose Asbury Park. The staff was carrying and attentive. the clinical Director, Jinky gave me her personal number and was always available to answer questions and guide us. Therapy team was great. They pushed Dad in the best way to be able to get him back home to all the staff at Asbury Park thank you for what you do every day you made a difficult time for our family much easier and we are very grateful!

I can't begin to express how greatful I am for the care and love that was provided at Asbury Park. This facility has staff that show true compassion for their residents. The nurses were polite and caring and the management was always around when I had a question or needed support. People are always smiling and I really enjoyed the time I spent. Now that I am back home and recovered I actually miss being there at times. Thank you for all that you did for me Asbury Park Team.

Our dedicated transport van helps residents get to important medical appointments—including dialysis, cardiology, oncology, and wound care—so they can receive the care they need without interruption. We also provide transportation to Room & Board and Board & Care facilities, giving residents greater flexibility and choice when planning their next steps.
